Painful urination typically makes me think of either a bladder/kidney infection or a sexually transmitted disease/infection. Bloody urine is consistent with bladder/kidney infections although it can also be due to a kidney stone. The presence of fever & back pain differentiate kidney infections from simple bladder infections (both of which are known as urinary tract infections or UTIs).
