blisters on thumb that itch and hurt all the way up my arm

for 15 years i have been getting blisters on my thumb every so often. they hurt and itch, allways in the same place on the middle knuckle of either thumb. Or on my schoulder, the doctor dosn't know what it is and it really hurts to the point it is hard to use my hand for days. If i take an antibiotic they go away faster, if I pop them the lisquid is clear it is painful but seems to make the extream itching stop. The blisters are clustered and small dots. Severel at a time.
Any Idea what it is and how to treat it?
