abdominal pregnancy - what is the usual mangement?

"My friend" has had a hysterectomy, with one ovary removed. ?A hole left in the vagina? allowing for fertilization with the remaining ovary? Mostly she's told she's crazy. Maybe 7-8 months along now, and not finding a physician who will do a laparoscopy, she's in a lot of pain, and quite discouraged. There are write-up from countries outside the US about abdominal pregnancies, but where can we find a physician who will engage her.
