I know that my dog needs surgery, but, should I have him go through that? It
has been a while and I am worried that he will not make it through the surgery.

Surgery on the diaphragmatic hernia have mortality risk, but you must take that risk if you like to have a healthy dog. But, it depend how severe hernia is. If the animal have physical disturbances, you must take that risk. It is not easy, but you should think and make that decision by yourself.
Mortality rises when the process is older than year and a half.
