Sleep Study Part II

The results of my Sleep Study are in...


Procedure: The test was performed by recording EEG, EOG, chin EMG, ECG, nasal and oral airflow, snoring sound, respiratory effort, oximetry, body position, pulse rate, and limb movement.

This study was performed in two stages. The first part of the night involved analysis of the patient's sleep pattern while breathing unassisted. After 150.5 minutes, the patient was placed on nasal CPAP therapy, and data was collected for an additional 266.8 minutes.


Sleep Architecture - Diagnostic: Without the CPAP device, sleep onset occurred at 9 minutes after the lights were turned off. REM sleep did not occur. The patient's total sleep time for this section was 121.5 minutes. The sleep efficiency index was 80.7%

Therapeutic: After CPAP was started, the patient's sleep onset occurred at 19.75 minutes after the lights were turned off. REM sleep did not occur. The patient's total sleep time for this section was 199.5 minutes. The sleep efficiency index was 74.8%.


Disturbances of Sleep - Diagnostic: While breathing unassisted, the minimum oxygen saturation during the study was 88%. The patient had intermittent snoring. The Apnea/Hypopnea Index (AHI) was 43 (less than 5 is normal). The total Periodic Limb Movement Index (PLMI) was 0. There were no significant cardiac abnormalities observed. Mouth breathing was noted.

Therapeutic: After CPAP was applied with the pressure of +11 cmH2O, the minimum oxygen saturation was 91%. With this pressure, the patient did not have snoring, and the AHI decreased to 1.9. The total PLMI on CPAP therapy was 0. There were no significant cardiac abnormalities observed. Intermittent mouth breathing was noted.


Impression: This recording reveals Obstructive Sleep Apnea (OSA) treated with a CPAP device.


After the follow-up I climbed into the car and wept. What started out as a slow stream of tears matured to venting sobs of frustration with a dash of hysterics. I had so many emotions crashing through me. And questions like, "Why the fuck didn't any of my other doctors, 'specialists,' Neurologists think of this before? Is it because they haven't truly listened? Is it because they'd rather push medications into the mouths of every living organism on the planet?" But wait...shame wants to join in on the fun - do I really want my husband to see me in that contraption? After the anger and and slight embarrassment subsided I felt elated because this could really be the answer I've been looking for.
