im addicted to nicotine patches and occasionally still smoke cigarettes too .. this has been going on for 7 years now .. HELP !!

i suffer from depression , anxiety , depersonalisation and derealisation and have had these mental health issues since i was 4 years old .. im now 34 ! ..
im on medication and see a therapist every week ..
