Summary:In a randomized, double-blind, placebo-controlled, 6-month, multicenter study involving 75 subjects with bipolar disorder (maintenance phase), treatment with N-acetylcysteine (NAC - a precursor of glutathione; glutathione depletion is known to exist in both depression and bipolar disorder) at a dose of 1 g/twice daily, as an adjunct to standard medication, was found to reduce depressive symptoms. A significant decrease in the Montgomery Asberg Depression Rating Scale (MADRS) was found (-8.05) 5 months after starting supplementation with NAC. Improvements in the Global Assessment of Functioning Scale and Social and Occupational Functioning Assessment Scale were found 2 months after beginning NAC supplementation. Considering the fact that treatment-resistant subthreshold depression is a major problem in bipolar disorder, the results of this study, suggesting that NAC may be an effective adjunctive treatment in patients with bipolar disorder, offer hope
