WHAT COULD BE THE REASON A BLOOD CLOT FORMS ON THE TOP SKIN OF A PENIS?

MY BOYFREIND AND I HAD JUST FINISHED HAVING SEX AND HE WENT TO TAKE A SHOWER WHILE IN THERE HE CALLED ME IN TO LOOK AT HIS PENIS IT HAD A VERY SMALL BLOOD CLOT THAT WAS THERE BUT IT WAS'NT BEFORE WE HAD SEX(ORAL AND PENETRATION).
