what causes weakness of the right thumb?

African-American Male Age 56, Tennis player with tennis or golfers elbow. muscle in right thumb is obviously smaller than the muscle in left thumb. He exercises it constantly but the weakness is still progressing. It appears to be developing atrophy. Please help! What should he do?
