What causes scrotal skin itching and what is the treatment for it?

My son has a burning sensation in his scrotum which makes him scratch the scrotum alot. This has been going on for close to one year and application creams and oitments recommended by a doctor has not helped. The itching has now spread to the area between the pubis and the thighs as well as backwards towards to anus.
