what causes an adult male to have puffy cheeks without any other obvious symptoms?

my brother is 25 years old he has had "normal sized" cheeks all his life, out of nowhere it seems maybe over the last 5 months his cheeks have become quite puffy looking...im worried it may be somthing serious..please help
