Varicose vein in my left testicle. Sperm countis 2 million.

I am a married Men since 2 years but still after many tries we haven't able to get child.

My wife tests are fine so Doctor said to have my semen analysis. I had a semen analysis which counts 2 million, then I had a ultrasound test where we found varicose veins in left testicle. Doctor said surgery will cure this problem.

Please suggest me that should I go for the surgery or is their any alternate available for my case?

 If surgery is fine then do you think the count will increase to 20 million which is necessary?

 If possible you can suggest which hospital/Doctor is good to undergo the surgery from him in Riyadh, Saudi Arabia.

Varicocele is the presence of varicose veins around the testicles. It occurs when blood does not circulate properly out of the testicles. The increase in blood leads to a raise in temperature in the testicles. This causes abnormal testosterone levels which hinder the production and maturation of the sperm. Although varicocele can occur in both testicles, about 85% of the time it affects the left testicle.

Varicoceles usually begins forming at puberty and most doctors do not screen patients during routine check-ups.

Surgical removal of the varicocele improves sperm level and function in 70% of the cases. Removing the varicocele has almost immediate effects.

Although a recurrence of the varicose veins will happen to approximately 5% to 20% of men, at least half the men who have surgery for varicocele are able to conceive a child within a year.

Alternative treatment option: Varicocele Embolization is a non-surgical procedure that is performed by Interventional Radiologists.  

Embolization does not involve general anesthesia, incisions in the scrotum, or sutures which surgery requires.  A patient with varicoceles on both sides can have both fixed with one vein puncture site (surgery requires two separate open incisions).  Embolization almost never requires an overnight admission in the hospital.   Studies show that embolization is just as effective as surgery.  Complications with embolization are very rarely heard of, but with varicocele open surgery fluid around the testicle (hydrocele) and infection can sometimes occur. Pregnancy rate studies for infertile couples show an improvement of approximately 60 percent after having embolization performed.

In a research study, Dr. Goldstein hypothesized, and later found that, the presence of varicoceles in the scrotum causes significantly lower testosterone levels. He also observed that following varicocele removal, testosterone levels are greatly improved—up to 100 percent—in more than two-thirds of the men studied.

Has your doctor checked the Prostate Gland also? Prostatitis, an infection of the prostate, can also affect the production of healthy sperm although this tends to only be temporary. With proper antibiotic treatment for the infection, prostatitis will clear up, allowing sperm production to return to normal.
