Swelling with a rash is a serious sign of inflammation and the likely hood that this is an allergic reaction, irritation or other condition.
Put ice on it to bring down the swelling and have him see a doctor or go to the ER for a correct diagnosis.

The area on his chest is swollen about as large as my hand (7 inches?) and in the middle is a 3 inch rash, splotchy red rash that looks like a big rug burn or "hickey". The rash is a few inches below his left nipple. However he has NO pain. We don't really have a doctor... is a visit to the ER really necessary if he has no pain?
