
Pain, reddeness, tenderness is a sign if inflammation and maybe a sign of infection.
The problem may be with his shoes. They may be too small or other problem in the heal area. Try getting insoles or new larger shoes.
If that doesn't work see a doctor for a correct diagnosis.

My boyfriend has presented with tender reddened areas in a half moon shape on the bottom of both of his heels. They have the appearence of stage 1 decubitis. they are not raised and acuallty seem slightly indented. The skin is not broken in these areas. They feel slightly warmer than the rest of his skin. The pain and reddness started about 1 week ago and has progressed to where he is in pain when bearing weight. His shoes are not new and no area in them seem to be applying pressure. He is 28, white male, slender, smokes tobacco, and is otherwise healthy
