New web app: Centor score

Added a new section to medwebapp.com: Respiratory , with the addition of the Centor score

Once again, this sort of web app project is very easy to generate with NSBApp studio . The app comrpises only one form, consisting of a Checkbox control, a Selector control and HTMLview output. The code looks like this:
