pain when erected, bleeding out of penis, yellowish discharge, pain when urinating

Your symptoms are of Chlamydia (The Clap), Gonorrhea, prostatitis or some other STD or like a “normal” bacterial infection of the penis from the urethra (UTI) . Normal meaning not sexually transmitted penile problem. Bacteria are all around us.

If it is a STD you have, all sex partners should be evaluated, tested, and treated.
Persons with a STD should abstain from sexual intercourse until they and their sex partners have completed treatment, otherwise re-infection is possible.

I would suggest you visited a urologist doctor for a exam, lab test screening and have this treated. 

Chlamydia and Gonorrhea can be easily treated by antibiotics if found in time.
