Could be male Performance Anxiety disorder symptoms (when guys have trouble "getting it up") can happen whereby he does get a penis erection,
and as he's about to have sex, he will lose the erection,
or it can be when he's actually in the course of having sexual intercourse, he will lose the erection, or he dosen't get erect at all.

Although orgasmic headaches are very painful, and obviously limit sexual enjoyment, they present no other acute threat. They are thought to result from muscle contraction, and/or blood vessel dilation, or elevated blood pressure in the head and neck during sex.

He should avoid or cut down on his masturbation as this can reduce libido (sexual desire).
