Models of Care for Costly Populations

The  June 2012 report from the California Improvement Network captures successful care models for complex and costly populations, such as the elderly and the frail.  CareMore's results, as compared to the general Medicare population, are impressive and include:

  • 80% of members participate in a Healthy Start visit
  • lower hospital readmissions (13.6% vs 20%)
  • lower per capita spending per (15%)
  • fewer hospital (63%) and SNF (67%) bed days
  • 97% are "somewhat" to "very" satisfied with their experience 

HealthCare Partners results include:

  • high quality ratings (clinical and patient experience)
  • low hospitalization rates
  • see need to better integrate palliative care and residential hospice
