Those of you who are working to comply with JCAHO's National Patient Safety Goals and the Institute for Healthcare Improvement's 100,000 Lives Campaign might be interested in the two resources below:

Study Abstract:  Reconciling Medications at Admission:  Safe Practice recommendations and implementation strategies   

The MA Coalition for the Prevention of Medical Errors Toolkit includes example tools such as forms, policies, staff and patient education materials to support adoption of the safety procedures.
