FOR IMMEDIATE RELEASE - June 16, 2011 - Bariatric Advantage of Irvine, California is recalling individual sample packs, and 60 & 180 tablet bottles, of its Chewable Multi Formula in the chocolate mint truffle flavor, because it may contain milk protein not listed on the label. People who have an allergy or severe sensitivity to milk may run the risk of serious or life-threatening allergic reaction if they consume these products.
No illnesses or adverse events relating to these products have been reported to date. The recall was initiated after it was discovered that Chewable Multi Formula high in vitamins A, D, E & K (Chocolate Mint Truffle) containing milk protein was distributed in packaging that did not identify the presence of milk. Subsequent investigation indicates the problem occurred because the milk allergen was not disclosed in the ingredient specifications supplied to Bariatric Advantage by the manufacturer.
Chewable Multi Formula high in vitamins A, D, E & K (Chocolate Mint Truffle) was distributed nationwidein bariatric surgery offices and by direct delivery to pre- and post-operative bariatric surgery patients.
