25 yr old woman; chills and body ache, one ear aches (behind ear), stomach ache, burning urine for 2 weeks

25 year old woman; has had burning urine for a week or two; now is miserable with chills (freezing), stomach ache and bad ear ache (actually says behind her ear). Doesnt' want to take Z Pack as had history of yeast infection ???
