What is the differential diagnosis and what further investigations should be done?

Patient: female, 48years, 60kg, 165cm.

Hypothyroidism on eltroxin 150mg/100mg alternate days. Thyroid level made yesterday- normal and good 

Recent Echo revealed : Mitral prolapse and trivial mitral regurge, everything else is normal. Recent ECG- normal.

Complain: Oedema, recurrent chest pain, tachycardia and dyspnea

What further investigations should I make? What could be the differential diagnosis?
