Could be  heat rash from excessive sweating, poor hygiene, infection or other skin condition.

Symptoms are also of:
1.  Candidiasis
 Fungal STD yeast infection of moist areas like genitals...
2. Contact dermatitis
 Skin reaction night clothing and to an irritation...
3.  Herpes
 Virus with one subtype causing cold sores and another causing genital herpes...
4. Psoriasis
 Chronic skin rash condition, usually red and blistery....
5. Scabies
 Mite infection of the skin common in institutions...
6.  Jock Itch

Try one of these Anti-itch creams and lotion. Look for over-the-counter (OTC) remedies with these ingredients:
    * camphor,
    * menthol,
    * phenol,
    * pramoxine,
    * diphenhydramine, and
    * benzocaine (or one of the other "caine" anesthetics).

Pain at the tip of penis is a sign of UTI or Std infection. You should go back to your doctor for  STD lab testing, and/or he can give you a differant prescription.
