At a very basic level, “no [Medicare] payment may be made . . . which . . . are not reasonable and necessary for the diagnosis or treatment of illness or injury or to improve the functioning of a malformed body member.” That clears it up, doesn’t it? Don’t worry if you’re confused at this point: you’re in very good company.
