Can something other than tobbaco use cause nicotine to show up in a urine test. Can a test have erraneous results?

Nicotine has shown up in a urine test done for an insurance co.  There has been no tobbaco use for 30 years and little or no contact with 2nd hand smoke.  Can anything else caus this, (an ingredient in a prescription drug etc.) Can these tests ever be mistaken?
