 What are the serious side effects of the nicotine patch and removal of it from an elderly long term chain smoker?

After being admitted to an assisted living facility that allowed some monitored smoking, the facility decided to try and stop the client from smoking completely with the aid of a nicotine patch over a three month period. The client with mild dementia had ceased requesting one of the 3 cig. allowed per day most days and the patch strength was lowered from the 14mg level to the next nevel (after 2 months). One month after, it was removed. The next day the client began having heart problems with the diagnosis of heart blockage 18 hours later and died the following evening --- a little over 48 hours from the removal of the patch. Could the heart have been stressed because of withdrawal? A ekg and echocardiogram had shown no problems 3 months earlier when she entered the facility and had not exhibited other symptoms or complained of problems.
