I have reflux and all that goes with it, i really think my problem is in my small intestine, any idea what it is

I have a very large stomach.....my limbs are small, I have had colonoscopies, and recently had an endoscopy (gastritis, duodenitis, bile in stomach, haetal hernia)....I am so uncomfortable, always constipated, never feeling relieve if I do go, my clothes don't fit.....this has gone on for years
