A low-fat diet that is well-rounded should be great. Adequate exercise that is appropriate for your health conditions are key, as well. I have found that many weight-loss diets with fancy names or that seem to be miraculous can be dangerous for the average healty person. Many recommend that you consult your doctor before trying them, especially with heart or kidney problems.
I have been under the care of licensed dieticians/nutritionists in the past for hypoglycemia and abnormally low weight. They take all of your health issues into account and usually provide a very detailed eating plan. There is no easy answer to managing your diet, but prudence and patience is usually the best way.
Good luck!
