What can having Mediastinal lymph nodes including prominent AP window lymph node measuring 1.6 x 1.4 mean?

Recent CT chest scan for chronic thoracic back pain for 3 months. Results Include Emphysematous changes and also right apical sup-pleural cyst measuring 2.7 x 2.6 cm. Was a former smoker pack a day for 20 yrs. I need some help interpreting this.
