What causes pain and internal bleeding (bruising) in the scrotum after exercising (without support) and scrotum numbness ?

Twenty years ago I began an intense workout routine in order to make my college basketball team.  I was out of shape and was really going overboard.  I did a lot of weight lifting and running (up hills etc...).  There was one thing I noticed that wasn't normal.  You know that leg machine where you lie on your stomach and curl your hamstrings?  I was on that straining a lot on my last curl and felt something move in my hip and low back area - but had no pain. 

The next morning I had internal bleeding (bruising) in my scrotum and my left scrotum area was in intense pain.  It was so bad, that I could hardly walk for 2-3 weeks.  Doctors couldn't figure it out.  Hernia tests when I coughed came/come back negative.  Ultra sound pointed out a thickening of the skin, but nothing else unusual. 

My symptoms are:

1)  left scrotum pain (this is the MAIN problem).
2)  left hip pain
*3)  When I have very irritated left scrotom pain, I have pain behind my left knee.  It feels tight and sharp.   
 
I have lived with this pain for 20 years now.  I have to always wear a jockstrap or I will start bleeding again.  I can't wear boxers.  I can't even sleep naked without being in pain.  I have no idea where the blood comes from if I exercise without support. 

A couple of new symptom have appeared.  I have more intense lower and now upper abbdominal pain.  And the one that is concerning me the most is - scrotum numbness.   I am a waiter and walk, bend, and lift all day.  I am finding that my testicles are going up and my scrotum is going numb and white.  This can't be good.  They aren't numb when I lie or sit down.  What's up with that? 

The sports I excelled in were basketball and volleyball.  I had a very good vertial jump (could dunk at 6 feet tall).  I have big thighs and I'm guessing there has been a lot of pounding on the pelvis area and have torn some muscles and tissues in these areas.  But where does this internal bleeding come from?  And why the numbness when I'm working?
