painful large bone infarcts in femur and tiba. with avascular necrosis. How is this treated and why is it soooo painful

I have had bilateral thr's due to avascular necrosis and now its in the femur and tiba with large bone infarcts. I have sheering severe pain in the mid thigh and shin splint type pain in my tiba. What are my options, treatments and pain control options. Thank tyou in advance.
