my left foot hurts at the big toe, inside ankle, heal and radiates up my leg through the shin up to my knee on to my hip,

this pain has been with me quite a while, in fact a few years ago I had it, mri showed nothing had physical therapy wore an air cast and a shoe insert, went away but its back with a vengance. Any suggestions?
