Chronic Chest pain around the heart, any info would be helpful.

Hello, I'm currently 19 and have been having chest pains around my heart for about 4 years now. When i first when to the doctor they said that it was a pinched nerve+muscle spasms and that it would go away within a few weeks, but it never did. I dont know if this is related but I have also had lower back pain since i was about 13 and its gotten worse over the years, now I cannot stand for longer than 30 mins to an hour or else my back pain becomes so bad that i cant breathe without intense pain. Any info would be very helpful.
