What could cause painful bowel movements in my 2 year old girl who is NOT constipated?

She is not constipated -- her stools are soft and she is fairly regular, but every other time she has a bowel movement, she screams and cries that her "poo poo hurts". Really cries.
She's been constipated in the past -- to the point where she had some blood on the outside of her stool, but it's been a while since that occured (maybe a month or more), yet her bowel movements still seem to bother her.
Her pediatrician did a urine sample to rule out a UTI last week and now is suggestion vasoline around the anus area, but nothing else.
