Could an anal exam of a toddler cause the toddler to have developed an external hemorrhoid?

My girlfriend's son was recently seen by a specialist b/c of a digestive/lactose intolerance issue. Her son was given an anal exam. Since the exam the child has complained about his bottom hurting and has an external hemorrhoid.
