Terry Pratchett is one of the most famous sufferers of Alzheimer's disease in the world. Mr Pratchett stated that he has started the formal process that could lead to his own assisted suicide at the Dignitas clinic in Switzerland.

"The only thing stopping me (from signing the documents) is that I have made this film and I have a bloody book to finish."

Pratchett is best-known for his popular and long-running Discworld series of comic fantasy novels.

Pratchett's latest documentary Choosing to Die explores the realities of assisted death. Pratchett will witness the assisted death of a patient named Peter first hand at the Dignitas Clinic in Switzerland.

Dignitas is a Swiss assisted dying group that helps those with terminal illness and severe physical and mental illnesses to die.

Additionally, they provide assisted suicide for people provided that they are of sound judgement and submit to an in-depth medical report prepared by a psychiatrist that establishes the patient's condition as fulfilling the specifications of the Federal Court of Switzerland.
