Gene expression profile for breast cancer responders to AI

Interesting article from J Clin Oncol looking at gene expression profiles in breast cancer patients responsive and resistant to the aromatase inhibitor letrozole. Of note is that tumor specimens were analyzed before and 10-14 days after treatment. I wonder if a new paradigm is emerging where breast cancers are not excised immediately after diagnosis but following sequential sampling after a variety of medical therapies. Surgery would be viewed then as a "last resort" for those tumors which are unresponsive to hormonal or other therapies.
