I would see an orthopedist.  Your chiropractor may be able to help as well.  I used to know someone in high school who's shoulder would dislocate occasioanally, he could sometimes get it to go back but I remember when it was bad he saw one of the town chiropractors.

Muscle work and deep tissue therapy like e-stim or ultrasound may help relax the ligaments and tendons.  If it is relaxed enough shoulder exercises can halp as well as prevent future shoulder dislocations by strengthing the area.
