started as a sharp pain in my left shoulder blade,resembling a pinched nerve,is now a constant dull, pain that doesn't go away

I've been having a pain in my left shoulder blade and upper arm for couple of weeks now. went to the doctor, he prescribed some cream and pain killers - they don't do the trick. It's getting really difficult to sleep and sit - standing up doesn't do any pain.any suggestions on what it might be and how to cure it? Thanks
