Pacemaker rhythms result in classical ECG with LBBB morphology.It is a universally understood fact that RV pacing would produce LBBB and LV pacing a RBBB pattern in surface ECG.As with any other rules in medicine , it is not 100% perfect .(May be 70%)

An 80 yo male presented with chest pain.
There is clearly a ventricular paced rhythm. Normally in a paced rhythm, the QRS is all negative from V1-V6 because the pacing wire is in the apex of the RV and thus all depolarization goes away from the apex.

If you've been following the Prehospital 12-Lead ECG blog for a while, you know that I'm advocate of using Sgarbossa's criteria to help identify acute STEMI in the presence of left bundle branch block (LBBB) or paced rhythm.

In pacemaker science , any pacemaker that maintains AV synchrony is often referred to as physiological pacemaker. This is of course , a wrong reasoning .None of the pacemakers available today can be claimed to be completely physiological .All pacemakers which paces the right ventricle induces IVS dysynchrony (Including the modern DDD)
