A 68 yo female with a history of HTN on metoprolol and of paroxysmal atrial fib presented with weakness. This is her ECG.
There is sinus bradycardia at a rate of about 55... in a p-wave.
She was also found to be hypothyroid.

I relaxed last night and had about 8 pints of beer, 3 large ice cream cones, some ripple chips and onion dip and watched UFC, then got to bed around 1:30 am.

The Holter Monitor is Back

It's been two weeks since Aiden was last attached to a Holter Monitor. Usually he gets his "leash", so to speak, once a month. Due to Aiden's new bradycardia episodes... and bradycardias. It is about the size of the older pagers.
What it does is basically a home administered EKG. It stores the recording until we return it to Dr. Cardiologist's office

bradycardic response occur .Many times the classical hypotension /bradycardia reaction is simply a reflection of heightened vagal tone.
Consequence of vagal tone on SA nodal and AV... rarely from both .
The common bradyarrhytmias that we encounter in inferior MI are
Sinus bradycardia
Sinus pauses ,SA blocks
AV blocks
Functional
Vagotonic
Organic
Ischemic

You are asked to see a patient with a pulse rate of 45 /mt . Is it sinus bradycardia or complete heart block ?
Only one condition , . . . you must conclude in the bed side !
Heart rate may give a clue ( HR of 30-40 is common in CHB . Less common in sinus bradycardia.)
Pulse volume is large in both (More so in CHB )
