Her surgeon might be in a catch 22 here:
If he does the full lift operation there are more scars. If he does the lesser operation, she may look distorted. A full lift is more risky for smokers which might also be a factor.

an article confirming that skin-sparing mastectomy with nipple areola complex preservation appears to be oncologically safe, provided the tumour is not close to the nipple and a frozen section protocol for the retro-areolar tissue
