An 81 yo woman presented at 2 AM to the ED with severe substernal chest pressure. This ECG was recorded
There is sinus rhythm with a PVC, and Right Bundle Branch Block... 0-1 mm of ST depression. Any ST elevation in V2 or V3 is very suspicious for LAD occlusion in a patient with chest pain.

tachycardia with left bundle branch block (LBBB). A previous ECG from one year prior was normal without LBBB. According to the ACC and AHA, new LBBB in the presence of ischemic... because, in their experience, most patients with new LBBB do not have coronary occlusion unless they also have some specific indicators of occlusion, known as the Sgarbossa criteria
