sould i tell my friend he has dementia

This is a matter of opinion. Mine is that "dementia" is a clinical condition best diagnosed by a qualified physician and, therefore, that this very important decision be made by that physician. If the professional advice is to tell the person he has dementia, I think the professional should be the one to do so. Generally this is no job for a friend.
I would take it back to the person who told you, if it was a family member of the dementia victim, it may be more specifically a duty for that family member.
