What could cause severe itching on palms of hands, bottoms of feet, and extensive wheals on legs after scratching?

This is driving me insane. I am 6 months postpartum (experienced excessive itching through entire pregnancy). My legs start with an itchy, crawling feeling, and when I scratch, large puffy lines appear and take hours to go away. Benadryl doesn't work anymore. I can't figure out what is causing this... I have not changed my diet, lifestyle habbits, etc.. I dust and vaccuum very often, and we don't have roaches. The palms of my hands and bottoms of my feet become itchy too, but i can't see a rash or hives. I have IBS, but my grandfather died from complications of Chronn's. Please offer any advice or possible avenues for me to explore. I feel desperate, the docs at urgent care just tell me to take antihistamines and shove me out of the room. Nothing helps anymore. I have more health conditions, but I figured this was a good place to start.
