spreading white spots on face and body

Since about 3 month now i have these feared white spots it started first on my right risk than it spreaded to my bakc and face their are very itchy i also became it on my breast aroung my nibbles since i ahve no medical  cant go see a doctor i tried cortison and antifungal but doesnt help can u help me please
