red itchy blotches on palms, wrists, ankles and feet

i have suffered from this condition for quite a while now and as the past year it has got intolerably worse... it starts off small itchy red spots and then turns into a coin size spot which swell feel raw and also feel very warm, to the point were i cannot walk because of the pain... can anyone help as i cannot seem to get this condition diagnosed or treated?????
