is frontotemporal dementia generally a clinical diagnosis? is there any way to conclusively make this dx?

my husband is 55 and has been unable to really leave the house or carry on normal activities because of paralysing apathy, rage outbursts, confusion, overwhelming desire to sleep, difficulty with carrying out plans or processing information (like putting together simple items or remembering our cell phone numbers). the neuro psych tests suggested a presenile progressive dementia consistent with ftd, but his ct scan shows only frontal lobe atrophy "within normal limits" of someone his age. his psychiatrist is advocating for a pet scan but our insurance company won't pay. any ideas?
